Appointment

We will make every effort to schedule appointments that are the most convenient for you and in return we ask that you make every effort to not change your appointment time once it is made for you. If appointments are missed without at least a 24 hour notice, you may be charged 50% of the planned procedure fee.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For all dental procedures completed in one visit such as fillings, extractions, and cleanings we require payment in full if you do not have dental insurance. For procedures which require two visits such as crowns, bridges, dentures and partials we require no less than 50% of the total at the first appointment and then the remaining upon completion. If these options do not fit your budget you may speak to our office staff for financing information through Care Credit.

If you have dental insurance, we require your co-insurance payment the day of service. Your portion will be estimated using the information provided to us by your insurance company. It is important to remember that we file your dental insurance as a courtesy. You are ultimately responsible for compensation for services we provide.
